FDG PET, dopamine transporter SPECT, and olfaction: Combining biomarkers in REM sleep behavior disorder

چکیده
BACKGROUND Idiopathic REM sleep behavior disorder is a prodromal stage of Parkinson's disease and dementia with Lewy bodies. Hyposmia, reduced dopamine transporter binding, and expression of the brain metabolic PD-related pattern were each associated with increased risk of conversion to PD. The objective of this study was to study the relationship between the PD-related pattern, dopamine transporter binding, and olfaction in idiopathic REM sleep behavior disorder. METHODS In this cross-sectional study, 21 idiopathic REM sleep behavior disorder subjects underwent 18 F-fluorodeoxyglucose PET, dopamine transporter imaging, and olfactory testing. For reference, we included 18 F-fluorodeoxyglucose PET data of 19 controls, 20 PD patients, and 22 patients with dementia with Lewy bodies. PD-related pattern expression z-scores were computed from all PET scans. RESULTS PD-related pattern expression was higher in idiopathic REM sleep behavior disorder subjects compared with controls (P = 0.048), but lower compared with PD (P = 0.001) and dementia with Lewy bodies (P < 0.0001). PD-related pattern expression was higher in idiopathic REM sleep behavior disorder subjects with hyposmia and in subjects with an abnormal dopamine transporter scan (P < 0.05, uncorrected). CONCLUSION PD-related pattern expression, dopamine transporter binding, and olfaction may provide complementary information for predicting phenoconversion. © 2017 The Authors. RAPID EYE MOVEMENT (REM) SLEEP BEHAVIOR DISORDER (RBD) INVOLVES COMPLEX BEHAVIOR AND A LOSS OF SKELETAL MUSCLE ATONIA DURING REM SLEEP. This parasomnia was first described in 1986 by Schenck et al.1 The minimal diagnostic criteria for RBD according to the International Classification of Sleep Disorders include movements of limbs or body associated with dream mentation and at least 1 of the foll...

REM sleep behavior disorder (RBD) is characterized by dream-enacting behaviors and unpleasant dreams, and presents a risk for self-injury and harm to others due to abnormal REM sleep during which control of muscle tonus is lacking (REM sleep without atonia) (Schenck et al., 1986; Schenck et al., 2002; American Academy of Sleep Medicine 2005). RBD is a heterogeneous disease entity consisting of ...

Introduction:Brain imaging with F-18 fluorodeoxyglucose (18F-FDG) positron emission tomography or Tc-99m hexamethylpropyleneamine oxime (99mTc-HMPAO) SPECT is widely used for the evaluation of Alzheimer's dementia (AD); we aim to assess superiority of one method over the other. Methods: Twenty four patients with clinical diagnosi...

Previous studies have shown that imaging with positron emission tomography (PET) and single photon emission computed tomography (SPECT) radiotracers that are specific for brain dopamine receptors can be used to indirectly image the change in the levels of neurotransmitters within the brain. Most of the studies in addiction have focused on dopamine, since the dopamine neurons that project to the...
